COLLEGE HOCKEY FEMALE SHOWCASE
LAS VEGAS, NEVADA

PLAYER:  Each player must have their own insurance to attend this hockey showcase.  Please submit a copy of your Insurance policy after registering.  Please fax to 306-732-4650.  Or scan and email to nd.lasvegas@notredame.ca

TEAMS:  Each player/team will be coached by a NCAA D1 or D3 College Coach.  The college coaches will run the on ice practices and instruct behind the benches for all games.  Each team will consist of a maximum of 2-3 goalies, 6 defence, and 12 forwards.  Once you have filled out the online registration and your fees are paid,  you are considered registered - if either of these are not complete you will be put on the waiting list. 

REFUND:  There will be no refunds after May 1st, 2012.  Before May 1st you will be charge a $150 admin fee if you with draw.

ON-LINE REGISTRATION:  A  form will open - make sure it is complete before you click submit.  This is the information that will be given to the college coaches.

MEDICAL:  There WILL NOT BE MEDICAL STAFF at the rink during this showcase.  But medical staff will be available on call.  Please be advised that ATHOL MURRAY COLLEGE OF NOTRE DAME DOES NOT CARRY ACCIDENT insurance for participants. Parents will be responsible for any medical expenses incurred while attending this Showcase.

NOTRE DAME waiver form:  Athol Murray College of Notre Dame is not responsible for any injuries that may occur at this hockey showcase.

VEGAS ICE waiver form:  Las Vegas Ice Center is not responsible for any injuries that may occur at this hockey showcase.
